1
Individual Transitional Case Management
This service is for those coming out of an acute care setting such as an inpatient setting, or addictions treatment. Re-entering your every-day life can be incredibly overwhelming when you are feeling raw and vulnerable.
Success happens when you feel supported and guided through the process of restructuring the old ways you have left behind. Case management offers you continued support for 90 days to help support your journey to recovery. This includes collaborative goal setting, weekly check-ins, virtual support, and guided connections to community resources to help increase your success.
2
Family Nursing Case Management
Sometimes healing and recovery involves families, caregivers or relationships that are also in need of healing, support and guidance. This is an extended case management service that includes extended family or caregivers who are also desiring to be part of the recovery process.
Collaborative goal setting as it relates to the family structure and culture, virtual support, group check-ins, and connection to community resources are included in this service.
3
Somatic Therapy Integrations
Specialized somatic, breathing, emotional-regulation, mindfulness, compassion-focused or other complementary techniques may be integrated into your support when they are clinically appropriate, and consented to by the client.
A client care plan is carefully curated based on your own unique recovery needs and is focused on clinical assessment, safety, care-planning and individualized care.
